The short answer
- Cerbo is strong where functional medicine is strong: 50+ lab integrations, a dispensary, and e-prescribing with controlled substances included for prescribing providers.
- Its cost adds up through per-role seats, a setup fee and add-ons for the patient portal, telemedicine and AI scribe, so model the first year, not the monthly line.
- OptiMantra (now the same company), Healthie and Practice Better are lower-cost general options. Aminova and Decoda suit prescribing-heavy and med spa practices.
- LongevityEHR is the Cerbo EHR alternative built around lab extraction, clinic-defined ranges and a portal branded as your clinic, with e-prescribing and lab ordering in development.
- Cerbo prescribing provider
- $281 per month, full-time
- Cerbo setup fee
- $1,195 one-time (a limited-time offer waives it for switching practices)
- Cerbo free trial
- Not listed, demo first
- Alternatives covered
- 7, including LongevityEHR
When Cerbo is worth it
Most clinics searching for Cerbo alternatives have already seen a Cerbo demo, and many of them should still buy Cerbo. It is a mature EHR for functional, integrative and direct primary care practices, and it is deep in the places those practices live: labs, supplements and prescribing.
Cerbo lists integrations with more than 50 labs, including specialty labs common in functional medicine, with results-only and bi-directional options so orders can go out electronically. It offers longitudinal tracking for integrated results, a dispensary for supplements, products and lab kits, an AI scribe, and a patient portal with secure messaging and lab results. E-prescribing, including controlled and compounded medications, is part of the prescribing provider plan.
In December 2025 Cerbo merged with OptiMantra under a single CEO. Both companies said they would keep offering their software, so for now they remain two products from one company.
Cerbo is a good fit if
- You prescribe daily, including controlled or compounded medications, and want it inside the EHR.
- You order heavily from specialty labs and want results to flow back electronically.
- You run an in-office dispensary for supplements and lab kits.
- You want structured onboarding, with six weeks of training and go-live support, and have the budget for it.

What Cerbo really costs
Cerbo publishes its pricing, which makes it possible to model honestly. Seats are priced by role, and several features that clinics assume are included are add-ons.
| Cerbo line item | Listed price |
|---|---|
| Prescribing provider, full-time | $281 per month |
| Non-prescribing provider, full-time | $256 per month |
| Supporting provider, full-time | $127 per month |
| Additional staff, full-time | $63 per month |
| Setup (server, 6 weeks of training, 2 weeks of go-live support) | $1,195 one-time |
| Patient portal | Add-on, $79 per month shown for 1,000 patients |
| Patient portal styling | $300 one-time |
| Telemedicine | Add-on, $27 per provider per month |
| AI scribe | Add-on, $29 per provider per month (60 notes) or $59 unlimited |
Put together, a single full-time prescriber who adds the patient portal (at the price shown for 1,000 patients) and telemedicine comes to about $387 per month, before the setup fee and any AI scribe. That can be good value for a practice using all of it. It is a lot for a small longevity clinic that mainly needs labs, plans and a portal.
What to look for in a Cerbo EHR alternative
- First-year cost. Seats, staff, setup, portal, telehealth and AI add-ons, for the team you will have in twelve months.
- Time to first patient. A six-week onboarding is right for some clinics and too slow for one opening next month.
- Labs you do not order. Integrated labs cover your own orders. Ask how each system handles a PDF a patient brings from elsewhere.
- Ranges and trends. Whether your clinic can set its own range per biomarker and sex, and whether patients can see the trend.
- Prescribing. Whether you need it in the EHR on day one, or can use a separate tool for a while.
- Brand and portal. Whether the portal is included, and how much of it carries your clinic's name and colors.
7 Cerbo alternatives to consider
1. LongevityEHR
LongevityEHR is a standalone EHR and white-label patient portal for cash-pay longevity, functional medicine, concierge and hormone optimization clinics with 1 to 5 providers. Where Cerbo starts from orders and prescriptions, LongevityEHR starts from the lab report. AI lab extraction reads a PDF or image from any lab, matches values to your biomarkers and holds them in a review queue: unit mismatches and low-confidence values are flagged, duplicates are caught, and the provider releases results to the patient.
Ranges are your clinic's own, per biomarker and sex, and biomarker tracking charts every value over time. The AI never interprets results: classification against your ranges is deterministic, and the provider writes the interpretation. The patient portal carries your logo and colors and is included in the plan.
- Live today: lab extraction, custom ranges and trends, care plans, SOAP notes with signing, intake forms, scheduling with video visits, multiple locations, bulk patient import, and memberships and payments through your own Stripe account.
- In development: e-prescribing and lab ordering are in development. Wearable data, an AI scribe, secure messaging and digital consents are on the roadmap.
- Pricing: Core $99 and Scale $199 per provider per month, unlimited patients, 30% off annually, and a 15-day free trial you can start without a sales call.
2. OptiMantra
Now Cerbo's sister product, OptiMantra is the lower-cost option from the same company: $99 per month for the first practitioner, $49 per additional practitioner, $25 per clinical staff member and free clerical staff. It serves several modalities, includes telehealth, memberships and electronic labs, and sells e-prescribing with controlled substances as a $38 per month add-on. It has a 15-day free trial and free standard data migration. See our OptiMantra alternatives page.
3. Healthie
Healthie prices by active clients instead of seats, from $19 per month for up to 10 clients to $129 for unlimited clients on Plus, and a Group plan from $149 with $50 per added clinician. Lab ordering runs through Fullscript and eLabs, e-prescribing through DoseSpot as a per-clinician add-on, and payment processing is on every plan. The 14-day Plus trial needs no credit card. Compare it in LongevityEHR vs Healthie.
4. Practice Better
Practice Better is a practice management platform with client programs and courses built in. Paid plans run from $35 to $155 per month, with portal branding and a personalized URL from the $99 Plus plan. Lab integrations include Rupa Health and Evexia Diagnostics, e-prescribing is a $49 per month add-on, and an AI charting assistant starts at $10 per month. There is no contract and a free trial on every paid plan.
5. Aminova
Aminova targets TRT, GLP-1 and peptide clinics, and also markets to longevity and functional practices. If your Cerbo shortlist was about prescribing depth, Aminova is the closest match: e-prescribing with controlled substances, prescriptions routed to named pharmacies, in-house dispensing with lot and expiry tracking, and phased protocol care plans. It starts from $99 per prescriber per month with staff free, through a demo.
6. Decoda Health
Decoda is an all-in-one platform for med spas and elective care that includes longevity clinics. It covers lab ordering with structured results, e-prescribing, an ambient AI scribe, an AI front desk, memberships and inventory, with month-to-month plans and no onboarding fees. Prices are not published. See LongevityEHR vs Decoda Health.
7. Heads Up Health
Heads Up Health describes itself as a clinical intelligence platform rather than an EHR. It unifies data from labs, wearables, medical devices and EHRs, so a clinic can keep its current EHR and add a longitudinal data view on top. Professional starts at $250 per month for 40 clients plus a $1,000 setup fee. It fits a clinic that wants to keep Cerbo for charting and add wearables data. See LongevityEHR vs Heads Up Health.

How to choose between them
Start with the work your providers do most. If it is prescribing and dispensing, Cerbo, Aminova and Decoda are built for it today. If it is a modest budget across several modalities, OptiMantra, Healthie or Practice Better will cover the basics for less. If it is reading lab panels, setting ranges, adjusting a plan and showing the patient their progress, look at a system built around that loop.
Then run a trial or demo on real material: three or four actual lab reports, one membership you sell, and the intake form you use now. The differences between these products show up quickly once your own data is in them.
Where LongevityEHR fits
LongevityEHR is for the small cash-pay clinic whose product is the patient's labs, ranges and care plan, sold as a membership under the clinic's own brand. It does not prescribe or order labs yet, and it says so. If that gap is acceptable for now, you can see pricing or start a free trial and upload your own reports today.
