When OptiMantra is the right choice
Before looking at OptiMantra alternatives, it is worth being clear about what OptiMantra does well, because for a lot of practices the answer is to stay. OptiMantra is an EHR and practice management system that serves several modalities: direct and primary care, behavioral health, med spas and IV clinics, functional medicine, and musculoskeletal and acupuncture practices.
Its pricing is published and easy to model. The first practitioner is $99 per month, each additional practitioner is $49, clinical staff are $25 and clerical staff are free. The base plan lists online booking, reminders, a patient portal with messaging, customizable charting, telehealth, compounding and IV workflows, point of sale, memberships and packages, electronic labs and basic data import. E-prescribing with controlled substances, insurance billing, two-way texting and a fax number are priced add-ons, and an AI charting add-on is listed too.
Getting started is self-serve. OptiMantra offers a 15-day free trial and, according to its FAQ, standard data migration from a previous system at no charge. In December 2025 OptiMantra and Cerbo announced a merger under a single CEO, and both said they would continue to offer their software.
Stay with OptiMantra if
- Your practice mixes modalities, for example primary care with IV therapy or acupuncture, and you want one system for all of them.
- You bill some insurance claims alongside cash-pay services.
- You need e-prescribing, including controlled substances, inside the EHR today.
- Price per practitioner matters more than longevity-specific workflow.

Why longevity clinics look for an OptiMantra alternative
The reasons are rarely about OptiMantra being a bad product. They are about the shape of longevity work. A longevity or optimization practice runs on repeated lab panels, ranges the clinic sets for itself, protocols that change as results move, and a membership the patient renews because they can see progress. A general practice system can hold all of that, but it is organized around the visit.
- Lab reports from many sources. Patients arrive with results from national labs, specialty panels and direct-to-consumer tests. Integrated labs cover what you order. Everything else is a PDF someone has to type in.
- Clinic-defined ranges. OptiMantra's own longevity content describes logging biomarkers, viewing trends and storing lab reports. If your clinic grades results against its own optimal ranges per marker and sex, check in a demo how each system handles that.
- Brand. OptiMantra's pricing page does not list a branding or white-label line. Clinics that sell a premium membership often want the portal to look like the clinic.
- The merger. OptiMantra and Cerbo are now one company, and both say they will keep offering their software. If you are planning for several years, ask in a demo how the two products will relate.
What to look for in an OptiMantra alternative
- How lab results get in. Integrated ordering, PDF extraction, manual entry, or all three. Ask what happens to a report from a lab the system does not integrate with.
- Who owns the ranges. Can your clinic set its own range per biomarker and sex, and does history keep the range that applied at the time?
- What the patient sees. Results, trends, plans, forms and payments in one portal, and whose brand is on it.
- Prescribing today vs later. If you prescribe daily, a system without e-prescribing today means a second tool in the meantime.
- Payments and memberships. Recurring memberships, bundles and credits, and whose merchant account the money lands in.
- Total cost. Seats, staff, add-ons, setup fees and per-client charges, not only the headline price.
- Getting started. A free trial you can run on real reports, or a demo first.
7 OptiMantra alternatives worth a look
1. LongevityEHR
A standalone EHR and white-label patient portal for cash-pay longevity, functional medicine, concierge and hormone optimization clinics with 1 to 5 providers. The core is AI lab extraction: upload a lab PDF or image, values are matched to your biomarkers, and anything uncertain is flagged for provider review before release. Biomarker tracking applies ranges your clinic defines per marker and sex, and care plans built from your templates show patients what to do next.
- Live today: lab extraction with review queue, custom ranges and trends, care plans, SOAP notes with signing, intake forms, scheduling with video visits, multiple locations, bulk patient import and a white-label patient portal.
- Payments: memberships, bundles, credits and one-off services through your clinic's own Stripe account.
- In development: e-prescribing and lab ordering are in development. Wearable data, an AI scribe and secure messaging are on the roadmap.
- Pricing: $99 or $199 per provider per month, unlimited patients, 15-day free trial.
2. Cerbo
Now part of the same company as OptiMantra, Cerbo is a long-standing choice for functional, integrative and direct primary care practices. It lists integrations with more than 50 labs, longitudinal tracking for integrated results, and a dispensary for supplements, products and lab kits. E-prescribing, including controlled substances, is included for prescribing providers.
It costs more to start: $281 per month for a full-time prescribing provider, a $1,195 setup fee (Cerbo runs a limited-time offer that waives it for switching practices), and the patient portal, telemedicine and AI scribe are add-ons. Our Cerbo alternatives page goes deeper.
3. Healthie
Healthie prices by active clients rather than practitioners: $19 per month for up to 10 clients, $49 for up to 250, $129 for unlimited clients, and a Group plan from $149 with $50 per added clinician. Lab ordering runs through Fullscript and Healthie's eLabs, e-prescribing through DoseSpot at $40 per clinician per month, and there is a 14-day trial of the Plus plan without a credit card. See LongevityEHR vs Healthie for a closer look.
4. Practice Better
Practice Better's paid plans start at $35 per month, with Plus at $99 for one practitioner and unlimited clients, and Team at $155 for two practitioners. Lab integrations include Rupa Health and Evexia Diagnostics, e-prescribing is a $49 per month add-on, and portal branding with a personalized URL starts on Plus. Trials run 30 days on Team and 14 days on other paid plans.
5. Aminova
Aminova is built for TRT, GLP-1 and peptide clinics, and also markets to longevity and functional medicine practices. Its strength is the medication side: e-prescribing with controlled substances, routing prescriptions to named pharmacies, in-house dispensing with lot and expiry tracking, and care plans for phased hormone and peptide protocols. Pricing starts from $99 per prescriber per month with staff free, and the path in is a demo.
6. Decoda Health
Decoda is an all-in-one platform for med spas and elective care, with longevity among the clinic types it serves. It offers lab ordering with structured results, e-prescribing, an ambient AI scribe, an AI front desk that answers calls, memberships and inventory. Prices are not published. Read LongevityEHR vs Decoda Health for the full comparison.
7. Heads Up Health
Heads Up Health is not a like-for-like EHR replacement. It describes itself as a clinical intelligence platform that unifies data from labs, wearables, medical devices and EHRs, and it lists EHRs among its data sources, so it usually sits alongside the system you chart in. That makes it an add-on rather than a switch: Professional starts at $250 per month for 40 clients plus a $1,000 setup fee, AI lab transcription is billed by usage, and white-label options sit in the Enterprise tier. See LongevityEHR vs Heads Up Health.

Switching from OptiMantra without losing history
Most of the risk in a switch is data, not software. Before you sign anywhere, ask OptiMantra for a full export of demographics, appointments, notes, documents and lab results, and check it on a handful of real patients. Then ask each alternative how it brings that data in and how long it takes.
In LongevityEHR, the patient list comes in through bulk import from a CSV or Excel file, and historical lab reports go through the same AI lab extraction and review queue as new ones, so past results become structured values in the patient's history rather than attachments. Plan for a short overlap where both systems run, and move the patients with upcoming labs first.
Where LongevityEHR fits
If OptiMantra is working and your clinic needs prescribing inside the EHR today, staying is a reasonable answer. If your practice has become a longevity practice, where each patient's labs, ranges and plan are the product you sell, LongevityEHR is built for that shape of work. You can compare plans and pricing, or start a free trial and put your own lab reports through it.
